In the last 3 years, I have experienced unexpected financial hardships: My son was diagnosed with [redacted] - monthly management expenses and medications, hospital visits, and doctor's visits; my wife has been diagnosed with [redacted] and [redacted] - therapist, psychologist, doctor, various [expensive] testing and labs; 2 car accidents - car insurance premiums, doctor's visits, medications; I was hospitalized for [redacted] for 1 week; I recently moved locations with my work - I made $25K less last year than the prior year(s); my daughter's braces; Hurricane Irma damage to my home; crushing credit card interest debt. All in all, these expenses were beyond my control and should not reflect any inability to properly manage my expenses or debt. Most of these new recurring bills surfaced after fixed expenses (purchase of a home, purchased cars, etc) were in place.

Also note this was reported later than I know is required due to not realizing it was a court case at the time - I had thought it was a pending settlement managed by a company I hired to settle the debt and did not realize until recently otherwise. I was planning on reporting the settlement once an agreement was made.
